You will receive a 2 nd DIGITAL WELLNESS FORM on the morning of your appointment to fill out as an added precaution
MODIFIED PAYMENT ARRANGEMENTS AND NEXT APPOINTMENT SCHEDULING When possible, to avoid delay and allow contactless exit from your appointment. In most cases YOU WILL BE ABLE TO SCHEDULE YOUR NEXT APPOINTMENT & EXIT THE PRACTICE DIRECTLY FROM THE TREATMENT ROOM.
PAYMENT OPTIONS will be limited to CREDIT/DEBIT & E-Transfer until further notice

MAIL & DELIVERIES - We have notified of all VENDORS and COURIERS of BACK DOOR DELIVERY for DECONTAMINATION as well as to not interfere with patients.

CLEAN ENVIRONMENT We have removed our coffee station, magazines, and items that can harbor or transfer germs. No food or beverages may be brought in.

SOCIAL DISTANCING MARKERS have been placed throughout our facility to help maintain social distancing practices.
